A free clinical screener you can put on your website.
The Treatment Readiness Tool is a structured, anonymous addiction selfassessment your visitors can complete in around five minutes. It is built on validated clinical instruments, gives them a clear result on screen, and emails you a copy if you want one.
Built on AUDIT (WHO 1993) and DAST-10 (Skinner 1982)
Clinically reviewed by the Bali Beginnings team
Anonymous by default. No data sold, no marketing follow-up.
What this tool actually does
The screener walks a visitor through four short modules:
1
Alcohol use
Ten questions from the AUDIT, the World Health Organisation’s standard alcohol-use screening instrument.
2
Drug use
Ten questions from the DAST-10, the most widely used drug-use screening instrument.
3
Withdrawal risk
eight questions that flag whether stopping suddenly might be physically dangerous (seizure history, recent benzodiazepine use, opioid use, and similar).
4
Daily life impact
Seven questions about how use is currently affecting work, relationships, finances, mental and physical health.
When they finish, the engine combines the four module scores into:
risk
level
A risk level on a five-point scale (no significant pattern → severe dependence)
Recommended level of care
A recommended level of care (no formal treatment → outpatient counselling → intensive outpatient → residential rehab → medically supervised detox plus residential)
detox recommendation
A detox recommendation (not required, evaluation recommended, or strongly advised)
Treatment
Setting
A treatment-setting recommendation that explains what each option looks like in plain English
They see a clean on-screen summary and can download a designed PDF of the full result. This is not a diagnostic tool. The PDF says so, the on-screen result says so, and the methodology page (this page) says so. It is a credible self-assessment based on published clinical instruments. It is not a substitute for a clinician.
What you get if you embed it
Three things, in order of how useful they tend to be:
1
A useful tool for your visitors
Most rehab websites already have a “do I have a problem?” page or quiz. Most of those quizzes are oversimplified and not actually clinical. This is the structured version. Your visitors get a result they can take to their GP, family, or a treatment team
2
Optional white-label branding
You can pass your own brand colour, logo, and organisation name. The widget on your page, the downloadable PDF, and the result email all theme to your brand. The only attribution to Bali Beginnings is a small line at the bottom: Developed and clinically reviewed by Bali Beginnings. We do not move that, hide it, or make it bigger
3
Optional Lead capture
You decide whether the tool sends results to you. If you provide an email address in the embed code, your visitors are asked at the end of the assessment whether they would like to remain anonymous or be contacted about their results. Either way, you receive an HTML email with the PDF attached. If they ask to be contacted, the email also includes their name, email, phone, and a message field. If they choose anonymous, you still get the assessment data; you just do not get a way to reach them.
Learn More
There is no monthly fee, no commission, no requirement to send referrals back to us. This is a tool we built for our own intake funnel and made available to other treatment providers because the bigger problem with online addiction screening is that most of what exists is not credible.
Three levels of integration
Pick the one that matches
what you want.
Drop the widget in. It uses our default styling. No emails, no white-label
Best for: blog posts, an “about addiction” page, an educational resource.
Drop the widget in. Set your own company logo. Set your own primary brand colour. A white-labelled and styled version of the screener is shown matching your style. Add an optional message presented to users with a link if you want a gentle prompter to reach out to you.
Best for: clinics that want the tool on their site for visitor engagement and credibility, but do not want options to capture a customer’s details (via opt-in) or a notification every time someone completes it
Same as Branded but including options to
a) send you copies of all results (including anonymous results which would not include customer details) and;
b) if the customer opts-in to be contacted about their assessment and provides their contact information, then that will be sent to you also
Best for: treatment providers actively taking new admissions who want to follow up with people who indicate they would like contact.
Privacy and what we store
This is the part your visitors care about and the part you should be honest about. Anonymous by default. Even without your data-organisation-email , completed assessments are stored on our server for service-quality and aggregate analytics. We do not collect a visitor’s name, email, IP address, geolocation, or device fingerprint at any point unless they explicitly opt in.
Substance selection (alcohol, drugs, or both) Each answer they picked Computed risk level, care level, detox and setting recommendations The embedder origin (the domain of the host page) A one-way hash of the user agent
Their email address (if they ask for their own copy) Their name, email, phone, and message (if they ask to be contacted by you)
Raw IP addresses, geolocation, device IDs, demographic data.
If a visitor contacts you to delete their data, give us their session ID and we will remove the row on request. We have no automatic deletion schedule yet.
FREQUENTLY ASKED QUESTIONS
Q1: Will my visitors know this is run by Bali Beginnings?
There is a small attribution line at the bottom of the widget. We do not move it. We do not make it large. You can use your own logo and colour everywhere else.
Q2: Can I customise the questions?
No. The questions are clinically reviewed and a change to one could invalidate the result interpretation. If you have a clinical concern about a question, email us and we will pass it to our reviewer.
Q3: Will you send marketing emails to my visitors?
No. We do not have a mailing list. The only email we ever send a visitor is the one-off copy of their result, and only if they opt in to it on the assessment
Q4: Can I see what the visitor sees?
Yes. Take the assessment yourself — same widget, against the live system. The session that creates is anonymous, just like any other.
Q5: Can the assessment run in another language?
Not yet. English only for now. If you have a serious volume of nonEnglish visitors and would benefit from a localised version, tell us — we may prioritise it.
Q6: Does this replace a clinician?
No, and we say so at the top of every result. It is a structured self-assessment that gives a clear picture of risk and a sensible recommendation. It does not diagnose, prescribe, or treat. The PDF flags crisis-support numbers for anyone whose result indicates immediate concern
